Overview

We are pleased that you and your doctor have selected us to provide services to you. On this page, you will find all the information you need to plan your visit with us. Below are general guidelines regarding your appointment, and several forms to be filled out prior to your visit.

A retina exam is more extensive than a routine eye exam that you would receive from your general ophthalmologist or optometrist. Your initial visit may take up to 2-hours but will depend on what diagnostic tests are deemed necessary based on your clinical condition. When your appointment begins, you will be asked about your complete medical history and any medication(s) you may be taking. Next, your eyes will be examined, including a vision check, an eye pressure check, and external examination. Both eyes will then be dilated for the retinal examination to further evaluate the back of your eye (retina). Following dilation, further diagnostic testing will be performed. Based on the diagnostic testing and evaluation, a management plan will be discussed in detail and we may treat you in the office during your visit.

What to Bring to your Appointment

  • Insurance Card
  • Photo ID
  • Filled out patient registration form unless submitted via portal
  • Your complete list of medications
  • A friend or family member to drive you home as your eyes will be dilated
  • Sunglasses to protect dilated eyes which may be sensitive to light

Insurance

To ensure we are in-network, please call the member services number on your insurance card.

Retina and Uveitis Center requests payment in full at the time of service. Patient responsibility includes co-payments, deductibles, and coinsurance. We accept cash, check, Visa, MasterCard, Discover, and American Express.

We will submit all insurance claims on your behalf if we are a participating provider. Required information includes:

  • A copy of your insurance card
  • Photo ID
  • An address and phone number to submit claims and verify coverage

We ask you to please review your insurance for deductible or coinsurance amounts that may apply at the time services are rendered. If we do not participate with your insurance, payment will be collected at the time of the office visit. Retina and Uveitis Center will not be involved in disputes between you and your insurance company regarding deductibles, co-insurance, covered charges, additional coverage’s, etc. We will supply requested information as necessary for further processing.
